To make a request, please fill out the information below, then select the "Update Hot Work Request" button. All information is required.

You should receive an email containing a link back to your completed request form at the email address you provide.

A member of the Fire Safety Office or their designated representative will come and inspect the site and your equipment. Upon completion of the inspection a paper Hot Work Permit will be issued. No work can start prior to being issued the paper permit.
